This Health Information Privacy Notice explains how Biohax LLC (“Biohax,” “we,” “us”) collects, uses, and protects health-related information in connection with our dietary supplement products and optional services. Biohax is a dietary supplement provider, not a healthcare provider. This notice supplements our Privacy Policy.

2. Health Information We Collect

We only collect health-related information that you choose to share, which may include:

• Health goals and preferences you enter when configuring products or completing an assessment
• Biomarker and epigenetic data from optional EET™ at-home testing kits, when those services are available
• Information you provide to our support team about your regimen

Providing health information is always optional — you can shop and use our store without it.

3. How We Use Health Information

Health information is used solely to personalize your supplement and nutrition recommendations and to provide the services you request. We do not use it for advertising, and we do not sell it.

4. How We Protect It

• Collected only with your explicit consent
• Stored separately from general account data, with enhanced access controls
• Encrypted in transit and at rest
• Never sold or shared with third parties for their own marketing

5. Sharing

We share health information only with service providers that help us deliver the service (for example, a laboratory processing an EET™ kit), under contracts that require them to protect it and use it only for that purpose, or where required by law.

6. Regulatory Note

If and when EET™ Guided services launch, handling of the associated health data will follow HIPAA and other applicable health-privacy regulations, and detailed consent will be obtained before any collection.

7. Your Choices

You may withdraw consent, or request access to or deletion of the health information you have shared, subject to applicable law. To make a request, email Support@biohax.com.
